Perhaps you want more control over your health care expenses. A Health Savings Account (HSA) plan helps you manage your medical bills using a tax-advantaged savings account. This option has one simple, calendar-year deductible per family.

Our Health Savings Account plans combine a lower-cost, high deductible health insurance plan and a savings account with important tax benefits. Pay qualified medical expenses with your account.

HSA plans can help you take control of your health, your medical decisions, and your medical spending. An HSA can increase your health insurance buying power.

  • Simple design: meet your deductible and the plan pays 100% of covered expenses for the calendar year.
  • Savings account you can use for qualified health care expenses or for retirement after age 65.
  • Tax deductible: HSA contributions are 100% tax-deductible from gross income up to IRS limits.

An HSA acts similar to the flexible spending accounts many have with their employer-provided health plans, except the money is yours to keep. There is no “use it or lose it” feature.

  • Deductibles include $6,300 per person and $12,600 per family.
  • No coinsurance.
  • Simple to use. Once the deductible is met, we pay remaining covered expenses.

Silver HSA 100(r)

  • Lower out-of-pocket costs and higher premiums.
  • Deductibles include $3,650 per person and $7,300 per family.
  • No coinsurance.
  • Simple to use. Once the deductible is met, we pay remaining covered expenses.

A copay plan is similar to many employer plans with a set copay for routine health care expenses. A copay plan can help you better budget for your family’s health care. When you or a family member has an office visit at a network provider, you’ll pay the copayment and we’ll pay 100% of any remaining history and exam fees. In addition, copay plans offer prescription drug coverage.

Bronze Copay Select(sm)

  • Choose from two different bronze copay plans.
  • Comes with a Preferred Price Card.
  • After the deductible is met, you pay a certain percentage of covered expenses.

Silver Copay Select(sm)

  • Choose from two different silver copay plans.
  • Includes a multi-tier prescription drug benefit.
  • Deductibles start at $2,500.

Gold Copay Select(sm)

  • Choose from two different gold copay plans.
  • Includes a multi-tier prescription drug benefit.
  • Deductibles start at $1,000.

Platinum Copay Select(sm)

  • Offers the lowest deductible of our copay plans.
  • Includes a multi-tier prescription drug benefit.
  • Out-of-pocket maximum is $1,500 per person or $3,000 per family.

 

Select Saver(sm) Plan

Consider Our Lowest Premium Plan

Our Saver plan saves you more on monthly premiums. This insurance plan is catastrophic coverage.

At least one qualification must be met in order to apply:

  1. Be under age 30.
  2. Complete the Certificate of Exemption to find out if you’re eligible for a hardship exemption, including whether minimal essential coverage is affordable.

  • Offers the lowest premium of all our plans.
  • Simple design: meet your deductible and the plan pays 100% of covered expenses.
  • Three in-network primary care doctor office visits (per covered person, per calendar year) for history and exam with a $35 copay.
