Silver HSA 100®

  • Low­er out-of-pock­et costs and high­er pre­mi­ums.
  • Deductibles include $3,650 per per­son and $7,300 per fam­i­ly.
  • No coin­sur­ance.
  • Sim­ple to use. Once the deductible is met, we pay remain­ing cov­ered expens­es.

When you or a fam­i­ly mem­ber has an office vis­it at a net­work provider, you’ll pay the copay­ment and we’ll pay 100% of any remain­ing his­to­ry and exam fees. In addi­tion, copay plans offer pre­scrip­tion drug cov­er­age.

Gold Copay Select℠

  • Choose from two dif­fer­ent gold copay plans.
  • Includes a mul­ti-tier pre­scrip­tion drug ben­e­fit.
  • Deductibles start at $1,000.

Platinum Copay Select℠

  • Offers the low­est deductible of our copay plans.
  • Includes a mul­ti-tier pre­scrip­tion drug ben­e­fit.
  • Out-of-pock­et max­i­mum is $1,500 per per­son or $3,000 per fam­i­ly.

 

Select Saver℠ Plan

Consider Our Lowest Premium Plan

Our Saver plan saves you more on month­ly pre­mi­ums. This insur­ance plan is cat­a­stroph­ic cov­er­age.

At least one qualification must be met in order to apply:

  1. Be under age 30.
  2. Com­plete the Cer­tifi­cate of Exemp­tion to find out if you’re eli­gi­ble for a hard­ship exemp­tion, includ­ing whether min­i­mal essen­tial cov­er­age is afford­able.

Plan Options:

  • Offers the low­est pre­mi­um of all our plans.
  • Sim­ple design: meet your deductible and the plan pays 100% of cov­ered expens­es.
  • Three in-net­work pri­ma­ry care doc­tor office vis­its (per cov­ered per­son, per cal­en­dar year) for his­to­ry and exam with a $35 copay.
